Job Summary
As the Physical Therapist in Home Health, you will be completing assessments and evaluations of patient care needs related to functional status/mobility, endurance/stamina, pain and other medical conditions.
You'll assist the physician in the development of the therapy plan of care,
provide services within the scope of practice as defined by state laws governing physical therapy,
treat patients to relieve pain develop restore function maintain maximum functioning using acceptable standards
of practice instruct inform participating members health care team patient family/caregivers regarding plan
care functional limitations progress toward goals complete all patient evaluations develops PT plan
carenote each visit submits progress notes per LHC policy reports outcomes evaluation goals anticipated projected frequency
careafterwards evaluethe patients response treatment changes condition supervising nurse physician.
- Evaluates the patient's functional status and physical therapy needs
- Assists the physician in developing a comprehensive treatment plan
