    Description

    Summary



    This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program.

    You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment.

    Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

    Duties




    Assigned Duties but not limited to:


    Accepts a shared responsibility for the overall management of clinic, including but not limited to developing and maintaining clinic grids, and sustaining clinic utilization.


    Accepts a shared responsibility in the development and ongoing review of all policies and operating procedures specific to clinical practice area.

    Responsible for identifying , measuring, and reporting relevant outcomes as related to specific clinical pharmacy practice.


    Contributes to budgetary stewardship by incorporating cost savings initiatives into clinical practice and identifying potential cost reduction opportunities to improve overall healthcare services.

    Independently resolves provider and patient care issues , concerns , and questions as related to clinical practice.


    Assesses tools and database to perform population management and high-risk patient identification , including identifying patients in need of comprehensive medication management , and recognizing patients who are being over- or untreated , or experiencing adverse drug reactions.

    Provides clinical coverage in the mental health pharmacist - run clinics.

    Offers expert knowledge and advance skills in pharmacology , therapeutics, drug-drug interactions and medication safety.


    Review clinic profiles for appropriate drug therapy , adherence issues, drug interactions, medication safety, unnecessary polypharmacy, and dosage adjustments for special.


    Obtains patient medication histories and counsels patients and /or relatives on medications as requested by providers or for patients enrolled in pharmacist- run clinics.

    Manage out-of-stock situations by substituting alternate dosage strengths.

    Identifies , reports, and appropriately manages medication -related complications , adverse drug reactions or events and medication errors.

    Provides expert, clinically appropriate drug information to patients , caregivers and medical center staff as needed.

    Maintains clinical experience , expertise, and scope of practice in areas consistent with clinical background.


    Effectively communicates with providers to establish positive interprofessional relationships, influence clinical care decisions, and solve technical issues related to clinical practice independently as needed.

    Verifies computer prescriptions for patients in his/her assigned clinic area or other areas as assigned.

    Adheres to pharmacy policies and procedures regarding prescription entry and dispensing of outpatient controlled substances.


    Maintains record contribution to pharmacy practice as defined by ASHP for preceptors and completes assigned residency assignments in a timely manner.


    Provide patient care that is appropriate to the cognitive , physical, emotional, and chronological maturation needs of the patient group served.

    Categories served include age 18-64 and older adults age 65 and greater.


    Establishes constructive relationships with both internal and external customers that elicit feeling and attitudes that promote positive relationships, communication and socialization skills.
