    Job Summary:


    The Sepsis Program coordinator is responsible for providing leadership, consultation, education, and patient care management expertise for patients with - at risk of developing – sepsis, severe sepsis, or septic shock.

    This includes leading and facility quality improvement and safety initiatives, the development and maintenance of a strategic plans and measure dashboards.

    The Sepsis Program Coordinator will collaborate with all members of the healthcare team in planning, implementing, monitoring, and evaluating the delivery of care to sepsis patients to achieve the best outcomes and patient experience, with a goal of reducing patient mortality.


    Duties & Responsibilities:


    On a daily basis, identifies potential septic patients and initiates measures to comply with the sepsis core measures; performs concurrent and retrospective data review.

    Regularly reports results and findings.

    Works with physicians, nurses, and other health care providers to implement sepsis protocols in a timely manner.


    Evaluates current clinical staff knowledge of sepsis protocols and based on findings, creates a robust program which includes education, monitoring, reporting, creating nursing order sets, and managing order sets through final approval processes.

    Monitors the sepsis program for compliance. Presents recommendations based on findings; implements performance improvement plans to improve sepsis patient outcomes.

    Advises hospital staff, medical staff and/or communities regarding opportunities and methodologies for improvement. Educates staff and details physicians regarding performance improvement processes/initiatives and patient safety.

    Attends and/or leads appropriate clinical team meetings to report findings from the Sepsis program.
    Demonstrates knowledge and understanding of current clinical practice guidelines, professional nursing standards, CMS regulations, regulatory/accreditation requirements, state licensure laws, national clinical quality benchmarks and organization policies/procedures; and applies this knowledge and understanding to the evaluation of patient care delivery;

    Develops ongoing Sepsis educational classes/training, including annual competencies, new grad training and nursing orientation.

    Requirements:
    Bachelor of Science in Nursing (BSN) degree or comparable degree program required
    Minimum of three years direct patient care experience in an acute care setting

    Preferred:
    Critical Care or Emergency Department experience.
    Three years quality/patient safety experience and/or management experience
    Current Rhode Island RN License and BLS
