    Description

    Our Client is seeking a Lead Social Worker for our to ensure that we continue to provide all patients with high quality, efficient care. We are an amazing team that works hard to support each other and are seeking a phenomenal addition like you who feels patient care is as meaningful as we do.

    Hours: 8:00-4:30 pm (rotating weekends)

    Job Duties

    • Plans, organizes, directs, defines and coordinates clinical services necessary to meet the legal, organizational, medical staff and regulatory agencies guidelines.
    • Provide case management, education and discharge planning. Performs community and educational activities.
    • Supports and coordinates the social service function for patients and their families, providing discharge planning pertaining to illness, hospitalization and financial concerns and facilitates post discharge planning
    • Works within the structure to negotiate and resolve problems and conflicts during the patient stay to ensure continuity of care.
    Qualifications
    • Master's degree in Social Work (MSW) required
    • Florida license required (LCSW - Licensed Clinical Social Worker)
    • Current BLS (Basic Life Support) required (within 30 days of employment start date)
    • Current CPI (Crisis Prevention Intervention) required (within 30 days of employment start date)
    • Minimum of two (2) years of experience in Social Services and/or Case Management experience preferred
    • Experience with Behavioral Health patients a plus
    • Successful Leadership / Management experience preferred
