    **Role Overview**



    Lifelong Access seeks a Residential Mental Health Professional (MHP) to provide case management and ensure services delivered align with each client’s person-centered treatment plan.




    • Service Delivery & Clinical Planning:


      • The MHP conducts Illinois Medicaid Comprehensive Assessment of Needs and Strengths (IM+CANS) assessments for assigned clients.

      • Treatment plans are developed, implemented, and maintained based on assessment results, client goals, and person-centered care principles.

      • Mental health interventions support clients meeting treatment goals through home visits.  Direct services include case management related to the treatment plan.  Timely service delivery ensures progress toward established objectives.  Progress monitoring informs adjustments to the plan as necessary.
