- Obtaining complete medical history and physical data on patients.

- Triaging patient telephone calls and providing consultation.

- Collaborating with a Medical Physician in accordance with professional guidelines.
- Participating in health education.
- Maintaining related records and statistical data and preparing statistical reports.
- Participating in interdisciplinary care conferences and assisting with referrals and dissemination of information to appropriate care partners.
- Assisting in the development and coordination of ongoing education and follow-up programs designed to measure client outcomes.
- Developing and maintaining affiliation with professional and community organizations.
- Assisting in development and compliance of clinical protocols, policies and procedures.
- Participating in quality review and improvement programs; monitoring quality and outcomes of care rendered.
- Performing other job related assignments and special projects as directed.
- Attends and participates in department meetings and is accountable for all information shared.
- Completes mandatory education, annual competencies and department specific education within established timeframes.
- Completes annual employee health requirements within established timeframes.
- Maintains license/certification, registration in good standing throughout fiscal year.
- Direct patient care providers are required to maintain current BCLS (CPR) and other certifications as required by position/department.

The knowledge, skills and abilities as indicated below are normally acquired through the successful completion of a Bachelor's degree in Nursing.
Two years of clinical nursing experience in practice area and successful completion of a Nurse Practitioner six month internship for new graduates or comparable experience is required.
Master's of Science Degree in Nursing is preferred.Attainment of prescriptive authority or successful completion of applicable course work and pass state board exam to obtain prescriptive authority within six months of hire date.
Bilingual speaking and/or writing skills are a plus.Knowledge & Skills
