Description

The LCSW works in collaboration with the team providing behavioral healthcare within scope of practice. Provides screening, assessment, diagnoses, evaluation, crisis intervention, treatment, consultation, care coordination and case management, education, referral, advocacy and other related outpatient services.

Job Description:

• Diagnoses psychiatric disorders and mental health conditions.
• Complete ASAM, LOCUS, Treatment Planning
• Distinguishes between physiologically and psychologically based disorders and makes appropriate diagnoses.
• Assesses patients' mental and physical status based on the presenting symptoms and complaints.
• Maintain confidentiality of records relating to clients' treatment.
• Encourage clients to express their feelings and discuss what is happening in their lives, and help them develop insight into themselves and their relationships.
• Guide clients in the development of skills and strategies for dealing with their problems.
• Prepare and maintain all required treatment records and reports.
• Counsel clients and patients, individually and in group sessions, to assist in overcoming dependencies, adjusting to life, and making changes.
• Collect information about clients through interviews, observation, and tests.
• Act as client advocate to coordinate required services or to resolve emergency problems in crisis situations.
• Collaborate with other staff members to perform clinical assessments and develop treatment plans.
• Evaluate clients' physical or mental condition based on the review of client information.
• Work in an interdisciplinary team environment.
• Lead treatment teams as needed.
• On call rotation for crisis.
