- Completion of an Accredited Licensed Nursing Program (RN)
- Minimum three years as an RN.
- At least one-year experience in MDS Coordinator RN role is preferred.
- Establishing MDS schedules and coordinating with the Interdisciplinary Team (IDT) for completion.
- Completing MDS per RAI manual, ensuring accuracy, timeliness, and validation.
- Transmitting completed MDS to CMS, reviewing reports for errors, and making necessary corrections.
- Reviewing hospital records to determine appropriate MDS ARD, projected HIPPS codes, and principal diagnosis for coverage.
- Leading PPS meetings, ensuring correct skilled coverage and care for residents.
- Supervising LVN MDS Nurse.
- Participating in Medicare and MDS webinars to stay updated with RAI and evidence-based practices.
- Collaborating with leadership for quality improvement initiatives.
- Performing other essential duties as needed.

The MDS Coordinator RN will be responsible for conducting and transmitting resident assessments (MDS – Minimum Data Set) in alignment with current State and Federal regulations. The role necessitates a deep understanding of MDS processes, involvement in utilization reviews, and proficiency in Quality Improvement and Care Planning.
