    Penn Presbyterian Medical Center has a full-time opportunity for a Clinical Pharmacist to join the Chemotherapy & Infusion Pharmacy Team.

    Responsibilities:

    • Evaluates drugs prescribed by physicians and providers for appropriate dose, therapeutic efficacy, allergy, drug interaction, age/height/weight/BSA, labs, tests, organ function, disease state, and contraindications, etc. Performs patient chart review, and references drugs and chemotherapy regimens with NCCN and/or treatment guidelines as appropriate
    • Corrects and/or clarifies any ambiguous, incomplete, or erroneous orders by direct communication with the physician and nursing team. Informs physician of nonformulary and restricted items as they are ordered and recommends appropriate substitution.
    • Performs final check on all prescriptions, ensuring accurate input, labeling, and dispensing; and provides counseling and drug information to patients and physicians.
    • Assists in the supervision of pharmacy technicians. Ensures the accuracy of their work prior to dispensing. Prepares and dispenses medications using appropriate techniques as needed
    • Assesses and maintains adequate medication and supply inventory levels for infusion operations, including navigating drug shortages/backorders
    • Participates in the management of medical emergencies.
    • Demonstrates the ability to cover all areas of the outpatient infusion pharmacy.
    • Responsible for counseling patients on their medication therapies and food/drug interactions as needed.
    • Maintains current knowledge of drug information, and all policies and procedures by participation in staff meetings, reading internal communications, etc. Assumes responsibility for adherence to medication-use policies and procedures.
    • Effectively responds to drug-related clinical questions to physicians/providers, nurses, and other hospital personnel with evidence-based answers, incorporating treatment guidelines, drug information resources, and clinical judgment.
    • Works well within an interdisciplinary team setting, utilizes problem-solving skills to provide solutions and uphold appropriate patient care and operations
    • Responsible for maintaining a current State of Pennsylvania licensure and for obtaining the necessary continuing education credits. Maintains a current knowledge of all advancements related to pharmacy practice and updates in oncology practice, through readings, hospital sponsored programs, continuing education, off-premises programs, etc.
    • Complies in all activities with accreditation, legal, regulatory, and safety requirements for a specific practice setting, including USP 797 & 800
    • Performs duties in accordance with Penn Medicine and entity values, polices, and procedures
    • Other duties as assigned to support the unit, department, entity, and health system organization. Actively participate in process/quality/clinical improvement initiatives and their implementation
    Credentials:
    • Current licensure in the State required.
    Education or Equivalent Experience:
    • Bachelor of Science or Doctor of Pharmacy Degree
    • Prior clinical oncology pharmacy experience preferred
