    Director of Clinical Operations


    Job Summary:


    The Director of Clinical Operations (DCO) for CareBridge is a vital clinical team member and is responsible for the ongoing design, execution, clinical oversight, and outcomes of the CareBridge 24/7 Member Support Program.

    The CareBridge 24/7 Member Support Program uses an interdisciplinary team to provide patient outreach, early detection of acute changes in condition or chronic disease exacerbations, and social determinants of health impacting care and interventions to improve patient outcomes.

    The population served are patients who receive Home and Community-Based Services (HCBS).

    The Director of Clinical Operations supervises and oversees the interdisciplinary team and team leads that work closely with patients, paid and family caregivers, health plan care coordinators, and other community providers.


    Responsibilities:

    • Actively engage in the interviewing and hiring of the interdisciplinary team
    • Train the interdisciplinary team members in the clinical model, role responsibilities, CareBridge policies and procedures
    • Promote a strong, high-functioning interdisciplinary team
    • Work closely with Sr. clinical leadership to continue to develop, enhance, and further evolve the 24/7 Member Support Program
    • Participate in the development of documentation templates, clinical guidelines, care plan guidelines and templates, and workflows
    • Provide clinical and operational oversight for the interdisciplinary team delivering services in the 24/7 Member Support Program
    • Responsible for both clinical and operational outcomesat levels determined by Sr. leadership
    • Communicate effectively with the entire team both within CareBridge (e.g., client services and business development) and with external clients
    • Participate in clinical case conferences between CareBridge and health plan care coordinators
    • Develop and implement training programs for internal staff, health plan partners, and employees of community providers
    • Lead quality improvement initiatives
    • Oversee team lead level managers
    • Deliver proactive, high-quality customer service
    • Problem solve and course correct when appropriate
    • Other duties as assigned.

    Qualifications:

    • Five + years of clinical operations or management experience
    • Five + years of clinical practice experience
    • Holds active, unencumbered Nurse Practitioner license in states of practice
    • Expertise in HCC, Coding, HEDIS, and RAF scoring
    • Multisite experience, operational oversight, and performance responsibilities
    • Experience in the care of adult, chronically ill patients
    • Advanced computer skills, including experience with Electronic Medical Records and Microsoft Office Suite
    • Expert communication skills, oral and written
    • Advanced proficiency in data analytics
    • Prior telemedicine experience a plus
    • Value-based-care and growth-stage startup experience required
