    Job Summary

    The Clinical Pharmacist Transition Specialists priority is to deliver optimal medication management across all dimensions of care at all times and ensure world class service recognized for excellence.

    The Transition Specialist is responsible for actively engaging patients, fulfilling prescriptions for accuracy and safety, and coordinating medication delivery and medication education to patients bedside prior to discharge.

    The Transition Specialist simplifies the discharge medication process, making it easy, so that patients can focus on what really matters - getting better and staying healthy.

    The Transition Specialist provides customer service excellence to all patients, creating unforgettable patient experiences.

    The transition specialist will work closely with individuals from pharmacy, nursing, and medical staff to ensure prescriptions are filled, delivered, and education provided on time before patient is discharged from the hospital.

    Coordination of efforts with nursing and physicians is critical.

    The clinical pharmacist Transition Specialist recognizes physicians and nurses as essential partners with shared goals of delivering the remarkable patient experience.


    The Clinical Pharmacist Transition Specialist will oversee Pharmacy Technician Transition Specialists, reconcile patient discharge medications, fill prescriptions, perform bedside medication education and make follow-up phone calls to discharged patients to validate medication education and to answer any questions patient may have following hospital discharge.

    Clinical Pharmacist Transition Specialist will be responsible for ensuring that all patients are discharged on appropriate therapy.

    The need for accuracy and precision in all aspects of the job is essential to ensure patient health and safety.

    During down time, the clinical pharmacist transition specialist is expected to work with inpatient staff and managers to support inpatient pharmacy operations.

    Minimum Qualifications

    Degree:
    Bachelors Degree in Pharmacy
    General Pharmacy Practice Residency
    PharmD preferred

    Licenses & Certifications:

    Texas State Board of Pharmacy Registration; In good standing; If licensure by reciprocity or if new pharmacy graduate, must obtain Texas license within the first 90 days of employment.


    Work Experience:
    Two (2) years related pharmacy experience preferred

    Management Experience:
    One (1) year management experience preferred

    Communication Skills:
    Exceptional Verbal (Public Speaking)


    Language Skills:
    Bi-lingual Spanish preferred


    Proficiencies:
    P.C., Typing 25 WPM
