    Director of Intake Opportunity Description

    Our client is a well-established provider of Home Health and Hospice services. They have a current opening for a Director of Intake based in the Worcester, MA area. Medicare-certified Home Health leadership experience is required. A clinician is preferred but not a necessity. Must understand intake processes, care coordination, and referral source management. This person will oversee the intake take and also develop relationships with local referral sources.

    Clinical Intake Director Job Requirements
    • Home Health Intake / Care Utilization / Referral Source Management experience.
    • Knowledge of Medicare Home Health rules, regulations including PDGM.
    • Clinician (Nurse or Therapist) preferred.
    • Track record of business development
    Clinical Intake Director Job Responsibilities
    • Day-to-day oversite of centralized intake department and clinical liaisons.
    • Coordinate proper placement of patients and appropriate level of care.
    • Communicate with discharge planners, physicians, referral sources, and patients.
    • Develop department / improve procedures and processes.
