    Provides patient centered pharmaceutical care by performing medication-related duties that involve prescription verification, processing, dispensing, consultation, and education. Assists pharmacy leadership with overseeing pharmacy technicians and ensuring regulatory compliance.

    JOB RESPONSIBILITIES

    Ensures appropriate medication storage, security and regulatory compliance in assigned area of practice

    IF ASSIGNED TO RETAIL/COMMUNITY PHARMACY ALSO RESPONSIBLE FOR:

    Adheres to all Federal and State Laws pertaining to dispensing and filling of authorized prescription medications and supplies, including but not limited to receiving and verifying prescriptions, selecting medication and filling prescriptions.

    Adheres to all departmental policies and procedures and regulations.
    Dispenses medication, supplies or equipment accurately and efficiently.

    Knowledgeable, compliant and adheres to regulations in dispensing control and non-controlled substances, including proper record keeping and documenting necessary monthly, quarterly, and annual inventory reports.

    Knowledgeable with 340B rules and regulations and ensures pharmacy maintains compliance.

    Assists customers with insurance issues, including obtaining prior authorizations and contacting physicians or other health providers on possible therapeutic interchanges.

    Maintains knowledge and facilitates third party insurance, including Prior Authorization process, Medication Formularies, Wellness programs, coupons, discounts, worker's compensation and other sources of payments, member's enrollment and eligibility.

    Provides consultation services regarding medication utilization and safety as appropriate for both prescription and over the counter medications and in guidelines of the California State Board of Pharmacy.

    Consults with other health care providers, information resources and payers to optimize therapy and maximize safety.

    Assists PIC with maintaining records and reporting in accordance to California State Board of Pharmacy prescription records, electronic records and faxes.

    Reports, corrects and maintains CURES information, Control Substances log, prescription records, Pharmacy specific records such as prescription logs, sales transactions, monitors receipts of payments including reconciliation and sales taxes.

    Assists PIC with adequate inventory control to process prescriptions in a timely manner.
    Reports any dispensing errors, medication errors or HIPAA violations in accordance to KD's policies.
    Provides consultative services to patients on medication related matters.
    Assists PIC in preparing necessary end of month reports, data analysis and other reporting needs as required.

    IF ASSIGNED TO HOSPITAL ALSO RESPONSIBLE FOR:
    Adheres to all departmental policies and procedures and regulations when assisting in meeting the ancillary, drug distribution demands of the hospital pharmacy department including delivering medications, accurately preparing and/or checking medications such as unit-dosed medications, emergency trays and carts, automated dispensing cabinet inventory, and checking active inventory for outdated medications
    IF ASSIGNED TO TRANSITIONS OF CARE ALSO RESPONSIBLE FOR:

    Participates in the transition of care process of targeted patient populations through the provision of services such as medication reconciliation, medication education, and care coordination in collaboration with the interdisciplinary team.

    Assists with measuring the clinical and economic effectiveness of the transition of care service provided with formal reporting of the data at defined intervals set by management.

    Assists with the development of policies and procedures to promote safe and effective use of medications during transitions of care.

    Guides, directs, and assists in trainings pharmacy technicians ,new pharmacist personnel, interns and/or residents and assumes responsibility for quantity and quality of their work.

    Documents interventions according to department procedures on an ongoing basis.
    IF ASSIGNED TO HOME INFUSION PHARMACY ALSO RESPONSIBLE FOR:
    Provides information about products and services offered by Home Infusion Pharmacy in a professional manner.

    Focus will be on Hospice and/or other non-IV compounding services and reviewing, processing, preparing, checking and dispensing medications for these patients.

    Inputs charges and ensures appropriate therapy reimbursement.
    Assists billing personnel as needed.

    Adheres to all departmental policies and procedures along with regulations when assisting in the drug distribution demands with scope of license and relevant competencies.

    Maintains medication, supply and equipment inventories and record.
    Conforms to the accounting and record-keeping of controlled substance mandated by state and federal laws.
    Provides after hour on-call coverage as assigned.
    Maintains an accurate and timely delivery service.
    Must be available to make patient deliveries as necessary.
    IF ASSIGNED TO CONDUCT PHARMACOTHERAPY EVALUATIONS OR ASSIGNED TO THE PHARMACOTHERAPY CLINIC ALSO RESPONSIBLE FOR:
    Conducts in-person or tele-health pharmacotherapy reviews with patients referred by provider.

    Provides timely and effective communication with referring prescribers and allied healthcare staff regarding patient visits and recommendations according to Kaweah Health policy.

    Is familiar with and utilizes the EMR to document patient visits and recommendations.
    Serves as the medication expert with regards to patient and provider education to optimize disease state management.
    Documents clinical interventions on an ongoing basis.

    Measures the clinical and economic effectiveness of the services provided on an ongoing basis with formal reporting of the data at defined intervals set by management.

    Works with patient and provider to optimize medication regimens to decrease adverse outcomes.

    Reviews medications to identify areas and opportunity for cost reduction and savings through therapeutic equivalent alternatives and/or through the organizations 340b program.

    Ensures regulatory compliance with patient referrals and with regards to the 340b program.
    Guides, directs and trains pharmacy technicians assigned as patient care techs to ambulatory care services.

    Participates in the development and implementation of policy and procedures, order sets and/or institutional guidelines to promote safe and effective use of medications.

    Serves as preceptor for pharmacy residents and interns.
    Adheres to all departmental policies and procedures.

    IF ASSIGNED TO ONCOLOGY/OUTPATIENT INFUSION CENTER:
    Processes medication orders and verifies ingredients prior to compounding.
    Confers with clinical pharmacist to clarify any discrepancies in the order prior to processing and compounding.
    Responsible for pharmacy technician oversight and quality of their work.

    Assures compounded medications meet standards for quality when supervising compounding, performing quality reviews of the finished product and completes recordkeeping in accordance with department policy and procedures.

    Accurately prepares and/or checks medications such as unit-dosed medications, emergency medications, automated dispensing cabinet inventory, and checks active inventory for outdated medications.

    Adheres to all departmental policies and procedures along with state and federal regulations as they pertain to sterile compounding and hazardous drug handling (USP 797/800).

    Conducts monthly med room inspections in assigned area of practice, checking for cleanliness, outdates, orderliness, and unauthorized drugs as required by The Joint Commission and Title 22 and keeps all appropriate records

    Knowledgeable of growth and development for all patient/family cultural, linguistic, spiritual, gender, and age specific needs. Able to effectively communicate and care for patient and family as reflected in the Plan of Provision of Care.
