- Assist in the daily operations, planning, priority setting, staffing and per diem usage for case managers
- Responsible for the orientation, establishment of a preceptor, and mentorship of new case managers.
- Identifies QAPI needs as appropriate; and contributes to QAPI data collection and process utilizing the PDCA model as directed.
- Responsible, in collaboration with DPS, to review, report, and follow up on all patient/family complaints.
- Reviews and interprets the daily answering service on-call log reports for the purpose of targeting areas for performance improvement.
- Collaborates with the interdisciplinary team to coordinate patient centered care, assist with management of symptoms,
- Responsible to make joint /follow-up visits in cases of uncontrolled symptoms, evaluation of staff competency, new staff education, complaints, questionable re-certification patients, in addition to the annual field evaluation.
- Responsible in concert with DPS for the oversight of approval and determination of appropriateness of change in level of care including inpatient admissions, continuous care, respite admissions, re-certification, and revocations from the program, followed by the completion of appropriate documentation.
- Responsible for the facilitation of the Dutchess and Ulster Team clinical meetings.
- Responsible for assuming weekly role as on-call supervisor on a rotating basis.
- In concert with the DPS ensures that all patients are appropriately evaluated for re-certification prior to their re-certification date and all appropriate paperwork is completed at the time of the re-certification process.

- Shows leadership qualities through effective communication
- All hospice settings including, home care, inpatient units, nursing homes, and normal office environment.
- Travel by car to hospice settings.
- Moderate to high risk to exposure of unsafe environments or unstable situations. Ability to carry up to twenty-five pounds of supplies and/or materials used to provide community education.
- Associate's from accredited nursing program, Bachelor degree in Nursing preferred
- Valid NYS RN License
- Five (5) years in which progressive clinical competence and management is demonstrated or comparable experience.
- At least 1 year of hospice experience
- Registered Nurse certified in Hospice and Palliative Care at time of hire or within two years of employment.

Overall responsibility for the clinical supervision and coordination of care for Dutchess and Ulster homecare teams.In concert with the DPS ensures the provision of high-quality hospice care is delivered in an efficient and cost-effective manner while maintaining compliance with the professional standards, hospice standards, federal and state regulations, JCAHO standards and NYS practice acts.
