    Job Title: Program Manager

    Conifer Park is seeking a skilled Program Manager to lead our Inpatient clinic in Glenville, NY. This role involves overseeing the development, implementation, evaluation, and reporting of program goals and objectives.

    Duties and Responsibilities:

    • Provide individualized care and attention to patients while managing patient census effectively.
    • Ensure timely resolution of patient concerns and complaints.
    • Maintain coordinated interdisciplinary care for patients before, during, and after discharge.
    • Coordinate delivery of specialized services from other relevant departments and disciplines.
    • Collaborate with Intake Department and Admissions Department for efficient patient admissions.
    • Supervise assigned staff, covering clinical skills and administrative issues.
    • Ensure adequate staffing levels and proper credentials at all times.
    • Develop and deliver mandatory trainings to staff.
    • Provide timely feedback, praise, and discipline to staff.
    • Continuously improve service quality to provide excellence in patient care.
    • Monitor and refine the clinical program to meet patient needs.

    Requirements:

    • Bachelor's Degree with QHP (e.g., RN, CASAC, MHC).
    • Master's Degree preferred.
    • License such as LMHC or LMSW preferred.
    • Basic PC skills desirable.

    Estimated Salary: $90,000 - $110,000 per year based on experience and location.
