Job Summary
The Lead APP will exclusively work in the in-patient setting supporting our Advanced Heart Failure Cardiologist.
Responsibilities
- Cordinates clinical and administrative process for patients through all phases of transplantation (pre, transplant and post).
- Educates patients, families and staff regarding transplantation.
- Coordinates with physicians and other program staff to evaluate and manage patients.
