    We're in search of a dedicated Registered Nurse to play a pivotal role in our Clinical Care Delivery Team. As an RN with us, you'll be instrumental in delivering top-notch, evidence-based care to a panel of seriously ill patients through telemedicine.     What you'll do

    Patient Care:

  • Manage patients assigned to your Care Team, ensuring the RN and Interdisciplinary team visit frequencies align with the patient's acuity level and needs.
  • Communicate with patients via phone and video conferencing as outlined in the care plan.
  • Provide patient care within your scope of practice and per provider orders.
  • Assess and enroll patients, completing initial referral follow-ups as needed.
  • Develop and update care plans based on ongoing assessments.
  • Manage patient medications, including implementing orders, maintaining current medication lists, assisting with medication refills, and and ensuring adequate supply.
  • Provide patient follow-up and education on new medication orders.
  • Facilitate requisition of medical supplies and durable medical equipment.
  • Complete care coordination activities, ensuring timely documentation updates and communication with primary care providers and other physicians.
  • Coordinate with the Interdisciplinary team and external agencies as necessary, including facilitating advanced care planning and hospice referrals.
  • Educate patients and caregivers on Vynca services, disease processes, symptom management, and medication tracking.
  • Perform additional duties as assigned.
  • Float Nurse:

  • Assist in covering patient care for other care teams during normal working hours.
  • Your experience & qualifications

    Willing and able to work Monday-Friday 8:30am-5:00pm in Pacific time zone, with the possibility of afternoons, evenings, and/or Saturday mornings

    Minimum Qualifications:

  • Current CA and OR RN license, with ability to obtain additional state licensure
  • 1 year experience in hospice or palliative care
  • Minimum of one year experience as a professional nurse within the previous three years
  • Excellent oral and written communication skills required
  • Preferred Qualifications
  • CHPN certification or equivalent
  • Compensation & benefits

  • $42- $52 hour: The actual hourly rate paid for this position will be based on several factors, including, but not limited to: your prior experience and skills related to the position, geographic location, company needs, and current market demands.

  • Organizational requirements
  • Works with Supportive Care patients remotely wherever they reside or are receiving care, including homes, skilled nursing facilities, hospitals and residential care for the elderly facilities.
  • Successfully performs the essential duties and responsibilities of the position through the use of physical activities as described below:
  • Regularly requires sitting, standing, walking, talking, use of hands, listening, and observing.
  • Regularly requires working at a computer for an entire shift (with appropriate breaks).
  • Requires use of video-conferencing technology.
  • Occasionally requires reaching, stooping, bending, kneeling, and lifting items weighing 25 pounds or less.
  • Occasional travel may be required to perform job duties, attend training, or company events and requires attendees to be fully vaccinated.
  • Works at all times within the policies and guidelines of the company
  • Plays an active part in the development and maintenance of good relationships with all who have business with the Company
  • Respects at all times the confidentiality of information covering patients, staff and volunteers and is HIPAA compliant
