    The Resource and Care Coordinator will work with youth and family to plan, implement and evaluate service provision and navigate treatment needs.

    Provide active care management including linkages to community resources including for needs in areas of social determinants of health. Ensure the coordination of support meetings and crisis plans for individuals served.

    Coordinate development of cross-systems crisis plans, intake/assessments, intervention and outcomes and any other applicable documentation of services provided. Participate in recurring meetings with leadership and ACT Team. Maintain linkages and relationships with community partners.

    The Assertive Community Treatment program serves youth ages 5-20 in Atlantic and Cape May Counties. The Program provides intensive and immediate access to treatment services and supports in youth and family's home and community.

    A multidisciplinary mobile team is tailored to each youth and their family and through a family centered approach will focus on crisis stabilization and assisting the youth with improving functioning and symptomology of mental health and emotional concerns.

    Interventions will assist with enhancing family functioning and overall stability in the family home.

    Full time, varies

    Education/Experience:

    Bachelor's Degree in social work, counseling, psychology, or human services field with a minimum of two years' experience providing services to people with mental health or co-occurring behavioral health challenges.
