Carry out clinical interventions and direct care visits
Diagnose and treat chronic illnesses
Monitor changes in resident health conditions
Work closely with physicians for medical management
Conduct Annual Wellness Visits and coordinate care with other healthcare professionals
Active Physician Assistant or Nurse Practitioner license in Texas
Current board certification in your specialty
Master's Degree in Physician Assistant Studies or Nursing
Preferably possess strong leadership, interpersonal, and problems-solving skills
Excellent verbal and written communication abilities
Punctuality and effective teamwork skills
