    JOB SUMMARY

    This position oversees the day-to-day healthcare operations of the community support services and directly supervises the health care given, assesses health, functional and psycho-social status of residents, initiates individualized service plans, proactively manages care and services for each resident, evaluates effectiveness and maximizes the resident's opportunity to remain in the assisted living environment. The Wellness Director oversees and promotes the residents' health and wellness through regular visits with each resident, resident assessment, and ongoing communication with the resident, resident's family and MorningStar care team.

    POSITIONOVERVIEW
    • Manage the staffing needs of the Assisted Living/Memory Care Community by interviewing, hiring, training, and supervising the Assisted Living Coordinator, Reflections Coordinator, and MCMs.
    • Manage the health care of all residents, including the dissemination of information to families, staff, physicians, and third-party providers.
    • Perform duties in accordance with established methods and techniques and in conformance with recognized standards.
    • Perform on-site assessments of residents admitted to hospitals or skilled nursing facilities for treatment and maintain contact with resident's family and discharge planner.
    • Complete pre-admission assessments for each resident and update every six months or as needed with change of condition.
    • Initiate resident negotiated service plans according to the individualized needs of the resident as prescribed by physician and/or community policy. Provide resident and family instruction. Where applicable, seeks input from the resident and family in the development of the negotiated service plan.
    • Perform ongoing assessments of residents' physical and psycho-social needs and coordinates with other departments to assure proactive quality care.
    • Order medications and oversee the resident's medication schedule for residents receiving medication administration services and assure that all medications prescribed are available by auditing the medication records and cart twice weekly and ensure proper documentation for medication assistance administration.
    • Provide coverage of the position in cases of emergency to ensure medication assistance is provided.
    • Ensure all infection control guidelines are met.
    • Knowledge of Assisted Living regulations, medication administrating regulations, and Occurrence reporting.
    QUALIFICATIONS

    • License/Certification: Current Nursing License issued by appropriate state licensing agency and must have a First Aid certification as required (LPN or RN).
    • Education: Minimum of 1 year of experience working with the elderly in an assisted living, skilled nursing or hospital environment preferred.
    • Continuing Education: As required by law and attend in-service educations within the community.
    • Job Knowledge: Knowledge of procedures and techniques involved in administering simple treatments and providing related resident-care service. Understanding of standard techniques used in providing personal services for resident and in caring for equipment and supplies. Good understanding of basis of universal precautions to avoid infection of residents and self or contamination of equipment and supplies. Be aware of the assessment process, and the development and execution of negotiated care plans. Possess the ability to solve problems and deal with a variety of situations according to MorningStar standards and the Nurse Practice Act.
    • Other: Must have the ability to read and interpret documents such as safety rules, operating and maintenance instructions, procurement manuals, etc. Must have good communication skills and be able to effectively communicate in English and understand and follow written and oral direction. Basic typing skills along with a working knowledge of personal computing and word processing software.
