- Develop comprehensive plans and strategies for establishing a successful home hospice program.
- Collaborate with senior management, physicians, and other stakeholders to define program objectives, policies, and procedures.
- Design protocols and workflows for patient admission, assessment, care planning, and discharge.
- Ensure compliance with all regulatory requirements, including Medicare, Medicaid, and accreditation standards (e.g., The Joint Commission).
- Stay abreast of changes in regulations and update policies and procedures accordingly.
- Oversee the preparation and submission of required reports and documentation.
- Participate in the recruitment, hire, and onboard qualified staff, including nurses and aides.
- Provide leadership, mentorship, and ongoing training to ensure staff competence and compliance with organizational standards and best practices.
- Foster a culture of teamwork, professionalism, and continuous improvement.
- Oversee the coordination and delivery of comprehensive hospice care services to patients in their homes.
- Conduct initial assessments and collaborate with the interdisciplinary team to develop individualized care plans.
- Monitor patient progress, symptoms, and medication management, and make necessary adjustments to the care plan.
- Provide direct patient care and support as needed, ensuring comfort and dignity at end-of-life.
- Establish quality indicators and performance metrics to assess the effectiveness and efficiency of the home hospice program.
- Conduct regular audits, reviews, and evaluations to identify areas for improvement.
- Implement quality improvement initiatives and monitor outcomes to drive continuous enhancement of services.
- Represent the organization within the community and build relationships with referral sources, healthcare providers, and community organizations.
- Collaborate with local agencies and organizations to enhance the continuum of care and support for hospice patients and families.
- Valid Registered Nurse (RN) license in the State of Missouri.
- Minimum of one year of clinical experience in hospice care, with demonstrated expertise in pain management, symptom control, and end-of-life care.
- Previous leadership or management experience in hospice, home health, or related healthcare setting preferred.
- Strong understanding of hospice regulations, reimbursement policies, and accreditation standards.
- Excellent communication, interpersonal, and leadership skills.
- Ability to effectively manage multiple priorities in a fast-paced environment.
