    Summary:
    This position assists the Clinical Coordinator and lead nurse with projects and processing, identifying, analyzing, and communicating quality data.

    The position is responsible for complete and accurate data collection, maintenance of records, and reporting for all patients who meet inclusion criteria for the American Diabetes Association standards of care accreditation.

    The position will review applicable patient medical records; identify, abstract, and analyze required data from medical records; enters collected information into appropriate databases, and submits timely patient data to the appropriate organizations and registries to maintain compliance.

    Communicates clearly and appropriate with the patient/family and other members of the health care team. Works closely with the Clinical Coordinator on all aspects of data management and other assigned projects. Plans work priorities in accordance to program goals. Engages full spectrum of communication excellence. Acknowledges and respects individual cultures, values, beliefs, and preferences.

    Education and Experience


    • High School Diploma or GED required.
    • 2 years of experience working with medical or population health required.
    • Knowledge of medical terminology is required.
    • Knowledge of Electronic Health Records (EHR) documentation required.
    • Must be goal oriented and the possess ability to maintain strict confidentiality.
    • Must have data entry experience and be proficient in using Microsoft office products.
    • Must be highly detail oriented, have the ability to remain focused, have good organizational skill, and excellent oral and written communication skills.
    • Preferred graduate of an approved licensed vocational nurse program.
    • Preferred experience with medical data abstracting.
    • Preferred experience with EPIC.
    Licensure, Registration, and/or Certification


    • CPR required.
    • Current licensure to practice professional nursing in Texas is required for candidates with the preferred LVN qualification.
