Position Description Position Title: Medicaid Unwind Community Health Worker Region 2 - Contract Position Supervisor: Medicaid Unwind Project Director Position Summary: The Medicaid Unwind Community Health Worker is responsible for conducting outreach, phone calls, targeted canvassing, and participating in community events to promote and support successful Louisiana Medicaid Unwind project expectations in the following parishes: East Baton Rouge, West Baton Rouge, Ascension, Iberville, Pointe Coupee, East Feliciana, and West Feliciana Parishes ("Region 2"). The position will assist participating Federally Qualified Health Centers ("FQHCs") in rendering outreach services and events in their designated region.Overview: The Louisiana Medicaid Program currently provides medical assistance to more than two million individuals throughout the State.
