Job summary
The Care Manager will participate in staff meetings and agency trainings as required. They will also complete 40 hours of trainings as required by OCFS. Additionally, they will participate in on-call rotation and monthly supervision with the program supervisor.Clients/Case Management Functions: Maintain daily face to face contact with clients. Complete daily apartment safety checks. Conduct monthly fire drills with clients.
The Care Manager is responsible for case record documentation and daily and monthly record keeping. They must also represent the Agency and Program in a positive manner.,
