    Description

    Medical Liaison - Licensed Practical Nurse

    SUMMARY

    We are seeking a skilled Medical Liaison - Licensed Practical Nurse to join our team at Harc, Inc. The successful candidate will provide medical support and guidance to individuals with developmental disabilities, ensuring their overall health and well-being.

    RESPONSIBILITIES

    • Assist the Program Registered Nurse (RN) and Director of Nursing (DON) in maintaining records and monitoring compliance with DDS agency policies and procedures.
    • Train support staff on medication administration, nutrition, universal precautions, and associated documentation requirements.
    • Monitor the overall health of supported individuals, including signs and symptoms of chronic and acute medical conditions.
    • Communicate current medical information to the Program RN, DON, medical providers, staff, and families.
    • Observe medication certified staff during medication passes and review monthly kardexes.

    REQUIREMENTS

    • Graduation from an accredited LPN school and current Connecticut LPN license.
    • One year's experience working with people with developmental disabilities.
    • Strong organizational and time management skills.
    • Ability to communicate effectively with individuals with developmental disabilities.

    CORE COMPETENCIES

    • Verbal and written communication skills.
    • Proficient computer skills, including Microsoft Word and Excel.
    • Ability to work as a team player and maintain consistent professional ethics.

    EQUIVALENT EXPERIENCE

    A valid driver's license and liability-free driving record are required.

    WORK ENVIRONMENT

    The work environment is active, requiring physical demands that may vary from light to heavy.
