- Develop and maintain collaborative relationships with area treatment providers
- Provide assessment, early engagement, and referral to those clients with co-occurring substance abuse diagnoses
- Complete substance abuse assessment for all clients admitted to program
- Participate in treatment planning with clients with substance abuse related goals
- Work with clients in obtaining needed resources and other general case management activities
- Act as member of the Individual Treatment Team for identified clients
- Regularly attend and participate in daily organization staff meeting
- Participate in on-call rotation as assigned
- Experience working with adults with serious and persistent mental illnesses
- Valid driver's license and ability to drive company vehicles
- Willing to work occasional evening and weekend hours
