    About the Role

    At Gulfside Healthcare, we are seeking a skilled and compassionate Weekend Hospice RN to join our team. As a Weekend Hospice RN, you will be responsible for providing quality care to patients with diverse needs, including those residing in private residences, assisted living facilities, and skilled nursing homes.

    Job Summary

    The ideal candidate will possess strong critical thinking skills, excellent communication abilities, and a commitment to delivering patient-centered care. You will work collaboratively with our interdisciplinary team to assess, intervene, and educate patients and families to maximize their comfort and enhance their quality of life.

    Key Responsibilities

    • Case manage the needs of assigned patients and families, ensuring timely and effective interventions.
    • Perform ongoing assessments of physical, psychological, social, and spiritual needs, initiating appropriate actions to address these needs.
    • Supervise and oversee P.N.'s, C.N.A's, and other assigned staff, providing guidance and support as needed.
    • Complete admissions and hospital referrals, collaborating with the Clinical Care Manager to ensure seamless transitions.
    • Support hospice inpatient units or crisis care cases, working closely with the team to address complex needs.

    Requirements

    • CURRENTLY LICENSED IN THE STATE OF FLORIDA AS A REGISTERED NURSE.
    • A MINIMUM OF ONE (1) YEAR'S NURSING EXPERIENCE IN A HOSPITAL, HOME HEALTH OR HOSPICE SETTING IS PREFERRED.
    • STRONG ETHICAL STANDARDS AND AN APPROPRIATE PROFESSIONAL Demeanor.
    • RELIABLE TRANSPORTATION TO PERFORM TASKS AND RESPONSIBILITIES IN A TIMELY AND APPROPRIATE FASHION.
