Job summary
The Case Worker - Substance Use is responsible for ensuring the provision of recovery-focused, person-centered care with a trauma-informed perspective.This position operates in a dynamic environment, and performs case management, assessment, and/or service coordination work based on the program's guidelines.
The Case Worker - Substance Abuse duties may include and are not limited to providing continuity of care services, linking consumers and families to all needed resources.
