Evergreen Village at Bloomington is seeking a Licensed Practical Nurse (LPN) to join our team

Responsibilities:

Administers medications as stated on physicianorders, ensuring strict adherence to Community medication managementdocumentation and procedure, and demonstrating steadfast dedication to residentsafety
Upon thorough assessment, creates personalizedservice plans tailored to individual care needs while championing residentindependence and dignity
Cultivates communication between physicians,healthcare providers, and family, fostering effective communication pertaining to the physical, mental, and social well-being of ourresidents
Contributes toward quality assurance andperformance improvement initiatives
Engages in continuous learning and developmentopportunities through attendance and participation in educational seminars,staff meetings, and other learning experience

Qualifications:

Current state licensure as Licensed PracticalNurse (LPN)
Current CPR certification
Exceptional communication and interpersonalskills
Must demonstrate the ability to serve with love,compassion, and dignity
