    This role involves two key responsibilities: serving as a palliative care consultant, offering specialized care directly to patients and their families, and also acting as a palliative clinician educator, aiding in the development of primary palliative care skills among healthcare professionals. These dual tasks are expected to complement each other and adapt as the palliative care program expands.

    Key Responsibilities:

    • 60%-70% Palliative Case Coordination/Consultation
      • Engage in and lead discussions on goals of care with patients at different stages of serious illnesses, employing advanced clinical communication techniques and a comprehensive approach. Tailor care based on disease progression, implement palliative care plans when necessary
      • Produce clear and informative documentation, especially for patients with complex needs requiring contingency planning. Apply best practices when utilizing documents like POLSTs, DPOA, among others
      • Collaborate proactively with colleagues from various disciplines and settings to create comprehensive care plans. Facilitate referrals to relevant organizations as needed
      • Collaborate closely with other providers to ensure coordinated palliative care case management, particularly for patients with multiple complex conditions or serious illnesses, especially towards end of life
      • Conduct visits with patients and their families in the Hospital
    • 20%-30% Education & Facilitation to enhance palliative care best practices within the clinical setting
      • Enhance competencies of providers and staff in advanced communication skills, managing serious illnesses, and the philosophy of palliative care; be a resource for palliative care across the organization
      • Provide both informal and formal education, under the guidance of the Palliative Care Manager, on topics like serious illness management, end of life care, effective communication, interdisciplinary collaboration, and more. Additionally, oversee Resident Physicians and Medical Students rotations, starting July, 2024
      • Participate in palliative care meetings, team huddles, and community gatherings as necessary. Engage in continuous professional development, particularly related to managing serious illnesses and providing palliative care through educational opportunities
    • 10% Other assigned tasks

    • Attend training sessions as mandated by organizational policies and state/federal regulations
    • Maintain regular attendance

    Requirements:

    • BA degree or equivalent
    • Graduation from an accredited nursing school
    • Basic Life Support certification for Healthcare providers
    • Washington State Registered Nurse license
    • Minimum 3 years of clinical experience
    • Washington State driver's license and vehicle for work-related travel

    Preferred Qualifications:

    • Master's degree or higher, and/or relevant clinical licenses
    • Experience in Palliative Care or providing care to patients receiving Palliative Care or Hospice services, involvement in goals of care, advanced care planning, symptom management, and end-of-life support
    • Experience with complex patients and interdisciplinary teamwork

    Knowledge, Skills & Abilities:

    • Knowledge: Proficiency in palliative care best practices, medical terminology, disease progression, and community resources. Familiarity with patient demographics and local values
    • Skills: Ability to creatively solve challenges in managing patients with complex illnesses. Strong communication skills to engage with diverse individuals and organizations. Proficient in relationship-building and influencing organizational culture. Exceptional organizational skills to prioritize tasks and meet deadlines with accuracy
    • Abilities: Adaptable with a high emotional intelligence, capable of working in a fast-paced environment independently and within a team. Ability to provide support and motivation to individuals from diverse backgrounds

    This role offers a salary range of $58,300 to $114,300 annually for Washington residents.
