    Registered Nurse RN / Licensed Practical Nurse LPN - Saint Francis Adult Day Health

    Demonstrates the ability to assess the health care needs of the participant, make a nursing diagnosis, transition this, through collaboration with the nursing team, into an interdisciplinary plan of care utilizing all resources available.

    Is able to communicate this plan to all care providers in a way that ensures continuity of care of the participant.

    Demonstrates the ability to establish and maintain functional communications with staff, other disciplines, participants, families, medical providers and community.

    Demonstrates the ability to lead an direct the nursing staff in the safe, effective delivery of care by providing a structured environment for staff and participants that promotes teamwork and participant safety.

    Demonstrates the ability to provide education to participants regarding their healthcare issues and care plan strategies.
    Provides ongoing education regarding nursing practices to other members of the healthcare team under his/her supervision.

    Demonstrates the ability to document data in an accurate and timely manner that maintains the organization's records to meet facility, state and federal requirements.

    Complies with legal requirements, standards, policies and procedures, including, but not limited to those within the Compliance Process, Code of Conduct, and HIPPA.

    At least two (2) years of clinical nursing experience, preferably in geriatrics or adult day health
    Working knowledge of electronic medical record.
    Able to speak, read and write in the English language
