    JOB SUMMARY

    Under the direction of the Family Health Supervisor, the Public Health Nurse (PHN) promotes and protects the health of the Carver County population using knowledge from nursing, social, and public health sciences. The PHN focuses on population health through continuous surveillance and assessment of the multiple determinants of health with the intent to promote health and wellness; prevent disease, disability, and premature death; improve neighborhood quality of life; and achieve health equity. These population health priorities are addressed through identification, research, planning, implementation, and evaluation of universal and targeted evidence-based programs and services that emphasize primary preventive interventions at all levels: individuals, families, communities, and the systems that impact their health. PHNs working with individuals and families do so within the context of a population focus, applying a systems perspective to factors that impact health. Population-level care targets systems, policies, organizations, regulations and laws by working with key stakeholders to influence health and social conditions to optimize the population's health. The PHN collaborates with other professions and stakeholder groups to promote and protect the health of those who live, work, learn, and play in Carver County. Advocates for interdisciplinary services, coordination of community services, and (at times) direct provision of services to assure all people have equitable access to high quality care and healthy environments. Acts in accordance with organizational goals, including emergency preparedness and response activities.
    TYPICAL DUTIES AND RESPONSIBILITIES

    • Identifies evidence-based programs and services to address priorities derived from ongoing, comprehensive, population-focused assessment, research, analysis and planning.

    • Accesses information sources using informatics and other information technologies to develop, implement and evaluate population health programs and services, and to enhance nursing practice.

    • Professional practice incorporates leadership, advocacy, collaboration, health teaching and policy development.

    • Applies a systems-level approach to assess potential or actual resources, identify health needs, opportunities, and inequities of individuals, families, and communities, to improve health outcomes of all populations.

    • Invokes strategies to address multiple determinants of health, health disparity, health promotion and health policy to improve health communication at all levels of health-related engagement.

    • Improves population health through collaboration with others such as: other members of the public health department, local and state human services agencies, community service agencies such as schools and law enforcement, businesses, faith-based organizations, healthcare providers, community coalitions and advocacy groups, and regulatory and policy-making bodies.

    • Participates in budgetary planning and control; grant application, development, management, reporting, and evaluation; and contract negotiation, administration, and management.

    • Participates in public health and organizational orientation and training of peers, colleagues, and others.

    • Ensures ongoing self-competency through goal-setting, self-evaluation, reflective practice and participation in peer review of nursing practice.

    • Adheres to and implements the following:

    - Principles of Public Health Nursing Practice (Quad Council of Public Health Nursing Organizations)
    - Core Functions of Public Health (Institute of Medicine)
    - Standards of Professional Public Health Nursing Practice (American Nurses Association)

    • Participates in emergency preparedness planning, training, exercises, and response in the event of a natural disaster, biological, chemical, radiological, or nuclear attack within the County, as well as infectious disease and/or public health threat.

    • Performs other comparable duties of a like or similar nature as assigned.
    MINIMUM QUALIFICATIONS

    • Registered Nurse's License with Public Health Nurse Certification in the State of Minnesota

    • Two years paid work experience as a four year RN and/or PHN

    • Basic Cardiac Life Support Certification

    • Valid Minnesota driver's license
    or

    • Equivalent combination of education and experience necessary to perform the essential functions of the position.

    PREFERRED EXPERIENCE BEYOND MINIMUM QUALIFICATIONS:

    • MECSH or other evidence-based home visiting program model designed to work with overburdened families who are at-risk for adverse childhood experiences, including child maltreatment

    • Work experience with women and children with special health conditions

    • Work experience performing surveillance, investigation, and prevention and control of infectious diseases, including case finding activities

    • Fluent in Spanish
