- Conduct assessments, develop care plans, execute patient care, and assess outcomes.
- Deliver skilled nursing services based on medical directives.
- Monitor patient condition changes and document observations.
- Utilize data to identify nursing diagnoses and create care plans.
- Active LPN/LVN license in the state of practice.
- Up-to-date TB or Chest X-Ray results.
- Current BLS certification.
- Previous experience with Tracheostomy and Ventilator patients is a plus.
