We are currently seeking a Nurse Practitioner PRN for our Hospice agency. The Hospice Nurse Practitioner works in collaboration with the Medical Director, patient physicians, and hospice team members, in an active practice to deliver, and support appropriate clinical care under Hospice federal, and state conditions of participation, and all Hospice policies and procedures to ensure excellence involving all aspects of the patient's clinical care.

Hospice Nurse Practitioner works directly with Hospice Clinical Director.,and Medical Director regarding assigned responsibility, oversight, and face to face encounters.

Essential Qualifications:

• Current RN licensure in the state of Ohio,and certification through a National Nurse Practitioner licensing board.


• Two years of Hospice, Home Health or Community Health within the past 5 years


• Must maintain such licensure in accordance with applicable laws and regulations and perform within the scope specified.
