    Job Summary

    We are seeking a skilled Registered Nurse (RN) to join our team as a Nurse Care Specialist. This is a rewarding opportunity for a motivated and compassionate individual to make a meaningful impact on patients' lives while working in a dynamic and supportive environment.

    Responsibilities

    • Conduct thorough patient assessments and develop personalized plans of care in collaboration with the healthcare team.
    • Maintain accurate and timely documentation of patient progress and care delivered.
    • Supervise and mentor Licensed Practical Nurses (LPNs) and Home Health Aides (HHAs).
    • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
    • Stay up-to-date with industry developments and best practices through ongoing education and training.

    Requirements

    • Current RN license specific to the assigned state(s).
    • One year of clinical experience as a Registered Nurse; less than 1 year requires approval by VP Clinical.
    • CPR certification and valid driver's license with liability insurance.

    Compensation

    The estimated annual salary for this position is $70,000 - $85,000, depending on location, job-related knowledge, skills, and experience.
