    JOB DEFINITION:
    Primary responsibility to maintain the security, care, use and disposal of all medication heldby the program, administer IM medications to patients, conduct basic physical exams onpatients and assist with improving the population health outcomes of the patients we serve

    EDUCATION REQUIRED:


    Bachelor of Science in Nursing from an accredited program*EXPERIENCE REQUIRED:New York State licensed Registered Nurse with substantial clinical experience withpsychotropic medications.

    Co-Occurring Disorder and medication assistedtreatment knowledge a plus.


    DUTIES AND RESPONSIBILITIES:
    1.

    Conduct basic physical exams on patients comprising of taking vital signs,measuring weight and height, as well as BMI and making appropriatereferrals.2.

    Accepts, registers, secures and discards medications in accordance withaccepted protocols.3. Maintain logs of all medications received in the Counseling Center/ARTS.4.

    Acts as liaison, in conjunction with the Office Manager, with the PatientAssistance Program for the purpose of securing medications for clients infinancial need.5.

    Reviews appointment schedules to ensure that program maintains sufficientquantities of injectable psychotropic medications to meet client needs.6.

    Establishment of liaison between this Agency and other medical andparamedical services used by patients so as to maximize exchange ofinformation and referrals.7.

    Any other support and follow up services appropriate to the role of a nurse inan outpatient behavioral health care which includes treatment of mentalhealth, substance use, physical health and co-occurring disorders inaccordance with Agency standards.8.

    Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress9.

    Complete QI Training during onboarding and participate in QI refresher training as needed10.Identify and implement quality improvement trainings and activities as part of professional development goals11.Encourage quality improvement processes and provide consistent resources for QI and professional development12.

    Ability to cultivate a culture of inclusion for all employees that respects their individual strengths, views and experiences.
