Job summary
We are seeking a RN Case Manager to join our team at Medical City Dallas. As a RN Case Manager, you will be responsible for promoting patient-centered care by coordinating the plan of care for the patient stay and managing the length of stay.Responsibilities
- Performs a comprehensive assessment of psychosocial, medical and discharge needs of patients/family along with an assessment of resources appropriate and available to the patient/family. ,
- Reassesses the patient's clinical condition as indicated. ,
- Care-coordinates all aspects of case management including but not limited to assessing high-risk patients under-going surgical procedures or other invasive treatments that may have post-operative complications related directly or indirectly due these conditions; ensuring timely interventions occur when necessary before any further damage occurs which could result in additional costs beyond what they need pay themselves out-of-pocket etc., without insurance coverage.
