RN BSN Home Visitor Bilingual

This is a comprehensive in-home community health nursing role providing services to women and their families enrolled in the Nurse-Family Partnership.

• Assesses physical, emotional, social, and environmental needs of women and their families as they relate to health and life course development.
• Provides counseling and instruction to assist women and their families in attaining targeted goals in areas including prenatal care, nutrition, parenting skills child healthcare family planning special health problems.

The Nurse Home Visitor will work with each family to establish realistic goals develop a plan for achieving them evaluate progress regularly revise plans accordingly collaborate with other professionals as necessary promote involvement of all household members support participation by other caregivers or medical providers offer education related resources available provide feedback on Visit Guidelines consider diverse cultural perspectives.
