    Mosaic at Home Home Provider Job Description

    • Provide support for daily living tasks such as cooking, cleaning, laundry and budgeting.
    • Help with skill acquisition, medication administration and medical appointments.
    • Support overall health and well-being by assisting with physical needs like bathing and toileting.
    • Daily documentation via web-based case management application (THERAP).

    As a home provider you will share your home with an individual requiring care. Your responsibility is to create a safe environment while helping them maintain independence through life skills development.
