- Coordinates all clinical pharmacy patient care and pharmacy production activities.
- Receive, review, and verify new patient orders with proper documentation and accuracy. Provides drug information, counsels patients, ensures adherence to state/federal licensure requirements/regulations, and maintains required paperwork.
- Compounds sterile admixtures and non-admixtures as needed or as assigned and/or determined by local/state pharmacy law. Performs all calculations for prescriptions, validates the Pharmacy Technicians' calculations as assigned, and ensures that all required paperwork is completed.
- Conduct and document initial and ongoing assessment of patients, their drug and disease state, and identify any real and potential drug-related problems or needs.
- Monitor the outcomes of drug therapy by collecting, evaluating, and interpreting the results of laboratory data, clinical data, and other pertinent patient information.
- Provide consultation and answer inquiries from patients, families, physicians and other healthcare professionals regarding medications and their proper use and storage.
- Interacts with heads of other departments, attends clinical meetings to monitor patient progress, and provides relevant pharmaceutical and infusion device information to pharmacy staff as required. Selects and evaluates the appropriateness of infusion devices for patients based on prescriptions and clinical assessments as assigned.
- Documents quality control measures and ensures that the pharmacy follows accreditation standards. Ensures adherence to pharmacy state and federal licensure requirements and regulations and maintains required paperwork.
- Adheres to the established pharmacy Quality Improvement and Control Program and develops the appropriate QI indicators based on trends, high volume, high risk, problem prone areas specific to each location.
- Process and check all medications, supplies and equipment prior to dispensing from the pharmacy.
- Maintain clinical skills and knowledge of pharmacotherapy by reviewing literature and attending in-services, seminars, and conferences.
- Requires minimal supervision and is self-directed.

Position Summary: Provides and coordinates pharmaceutical services for patients within the service area. Monitors, documents, and reports patient outcomes and provides or coordinates the dispensing of medications.
Essential Duties and Responsibilities: The essential functions include, but are not limited to the following:
Minimum Qualifications (Knowledge, Skills, and Abilities)
· Bachelor of Science in Pharmacy required, Doctor of Pharmacy preferred.
· Registered Pharmacist License in Texas.
· Knowledge of and Experience with compounding infused drugs and USP 797 Standards
· Proven record of evaluating and monitoring drug therapies for patients in the home.
· Good organizational and time-management skills.
· Participation and membership in professional societies and organizations is desirable.
· Excellent verbal and written communication skills.
· Flexible schedule with the ability to work on-call evenings and weekends on an as- scheduled basis.
