    Licensed Practical Nurse (LPN) – Home Health

    Field-Based | Full-Time | $2,500 Sign-On Bonus

    What You'll Do

    As a Home Health LPN, you'll provide skilled, compassionate care while helping patients remain safe and independent at home:
    Deliver nursing care based on the Plan of Care under RN and physician direction

    Provide patient-centered education and support for self-care

    Monitor, document, and communicate changes in patient condition

    Collaborate closely with RN Case Managers and interdisciplinary teams

    Promote patient safety and help prevent unnecessary hospitalizations or ER visits

    Build trusting relationships with patients, families, and caregivers

    Accurately document visits, medications, and outcomes

    Uphold the highest standards of professionalism and compassion


    Job Requirements:
    Graduate of an accredited nursing program (Certificate, Diploma, or Associate)

    Current

    State LPN/LVN License

    1 year of clinical experience

    (home health or hospice preferred)

    Comfort with independent field-based work

    Valid driver's license, reliable transportation, and insurance

    Sit, stand, bend, lift and move intermittently and be able to lift lbs

    Ability to travel (approx. 50%) and meet physical job requirements
