Qualifications & Responsibilities:
LSW or CSW license preferred

Maintain
accurate
, thorough, and

documentation.
Participates in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
A wide degree of creativity and latitude is expected
Review Advanced

ives and provide education to residents and family members as needed.
Help provide a smooth admission and discharge process for residents and family members.
Other duties as assigned by Administrator.
Prior experience managing staff is preferred.
Ability to work independently or part of a

Computer knowledge.
Previous
long term care experience is preferred but not
required
