    Description
    PURPOSE

    Provide oversight to the day-to-day operations of SWIFT (PSH/RRH) Community Housing and ensure fiscal and programmatic sustainability and oversight of the department.

    Job Duties:
    1. Serves as Team Lead to all SWIFT department programs at YWCA staff and provide support and leadership.
    2. Assist with the overall daily functions of the program and ensure staff is performing per program guidelines.
    3. Communicate with supervisor regularly to property address staffing and resident related concerns.
    4. Hold Staff accountable for performance and overall program quality
    5. Perform regular quality assurance audits as identified by the program supervisor.
    6. Responsible for submitting timely and accurate reports to supervisor.
    7. Provide case management and coordination service. This includes but not limited to consistent communication with the intent of supporting the household and addressing barriers to housing stability.
    8. Develop YWCA and community partnerships that can be beneficial to program participants.
    9. Develop a working relationship with property management. Expected to establish consistent communication with the intent of supporting the household and addressing barriers to housing stability.
    10. Enter timely and accurate data into HMIS (Homeless Management Information System) per coordinated access and YWCA policies.
    11. Develop goals, document all contact, income changes and outcomes in HMIS.
    12. Maintain household files per YWCA and funder standards.
    13. Communicate and case conference regularly with supervisor, and other outside organizations working with program participants.
    14. Link families to appropriate services and activities within and outside YWCA as identified in the program participant individualized service plan.
    15. Assist program participants with the re-certification process as needed
    16. Case Conference with property rental manager, providers, and other community resources.
    17. Participate in community workgroups/meetings as it relates to the Continuum of Care and coordinated access.
    18. Demonstrates understanding of and sensitivity to cultural differences, person centered practices and trauma informed care.
    19. Demonstrates compliance with all appropriate HIPPA Privacy and Security Standards as defined by the YWCA of Rochester & Monroe County

    RELEVANT BACKGROUND

    Experience and Education Qualifications

    § Associate's degree in Social Work, Psychology or Human Services related.

    § Three (3) years' experience in case management/counseling.

    § Minimum 1 year of experience in program supervision and working with mentally ill, MICA and substance abuse populations.
