    Description

    PURPOSE AND SCOPE:

    Responsible for providing nutritional services to in-center and home patients to boost their nutritional status and enhance clinical outcomes. Works collaboratively with the healthcare team to support quality enhancement programs and activities. Actively engages in process improvements to aid patients in reaching their specific goals set by their physician.

    PRINCIPAL DUTIES AND RESPONSIBILITIES:

    • Assesses patient's understanding of diet and kidney disease, offering tailored education on the relationship between diet, kidney disease, and treatment.
    • Delivers detailed education to patients and families on nutritional status, fluid levels, lab results, and disease-related parameters.
    • Conducts nutritional assessments and contributes to comprehensive interdisciplinary assessments.
    • Collaborates with the team to develop individualized care plans based on patient needs.
    • Determines diet prescriptions based on standard practices.
    • Documents patient progress notes according to guidelines.
    • Provides ongoing nutrition assessment and counseling to support patients' nutritional well-being.
    • Identifies malnourished patients and collaborates on interventions.
    • Monitors and adjusts nutrition therapy to maintain effective nutritional status.
    • Participates in recommendations for patient care and treatment plans.
    • Adheres to company policies and guidelines.
    • Participates in quality assessment programs.
    • Collaborates with team members on patient education.
    • Reviews and acts on nutrition-related lab results.
    • Communicates effectively with healthcare providers regarding patient care.
    • Attends staff and care plan meetings.
    • Consults with Dietitian on nutrition concerns.
    • Continues professional development.
    • Ensures a safe and clean work environment.
    • Performs assigned nutrition management duties and other related tasks.

    PHYSICAL DEMANDS AND WORKING CONDITIONS:

    • The role involves working in a healthcare facility with potential exposure to diseases.
    • Requires interaction with patients, staff, and travel to different locations.

    SUPERVISION:

    • None

    EDUCATION AND REQUIRED CREDENTIALS:

    • Registered Dietitian as per Commission on Dietetic Registration.
    • Board Certified Specialist in Renal Nutrition preferred.
    • Current state licensure if required.

    EXPERIENCE AND SKILLS:

    • Minimum 1 year of clinical nutrition work experience as a Registered Dietitian.
    • Previous experience in renal nutrition is a plus.
    • Demonstrated leadership, organizational, communication, and computer skills.
    • Able to work independently and with a team.
