Job Summary:
Aquor Technologies seeks an experienced Occupational Therapist (OT) to provide skilled rehabilitation services in a Skilled Nursing Facility (SNF) setting. The ideal candidate will have a strong understanding of occupational therapy principles and excellent communication skills to interact with patients, families, and a multidisciplinary team.

Key Responsibilities:

• Assess patients' physical and mental abilities to determine rehabilitation needs, focusing on activities of daily living (ADLs) and functional mobility.
• Develop and implement individualized treatment plans designed to help patients regain or improve their independence in daily activities.
• Provide therapeutic interventions to enhance fine motor skills, coordination, strength, and mobility, using a variety of techniques and equipment.
• Educate patients and their families on adaptive techniques and the use of assistive devices to enhance safety and independence at home.
• Collaborate with a multidisciplinary healthcare team to ensure comprehensive patient care.
• Monitor and document patient progress, adjusting treatment plans as necessary to achieve optimal outcomes.
• Assist in the discharge planning process, providing recommendations for continued therapy, home modifications, and support services.

Requirements:

• Strong understanding of occupational therapy principles, especially in the context of a Skilled Nursing Facility (SNF) environment.
• Excellent communication and interpersonal skills for interacting with patients, families, and a multidisciplinary team.
• Proficiency in creating and implementing individualized treatment plans based on patient needs.
• Active state Occupational Therapist (OT) License in good standing.
• A minimum of 2 years of experience as an Occupational Therapist or in a similar role.
• Certification in Basic Life Support (BLS).
• Certification in Advanced Cardiovascular Life Support (ACLS).
• Experience with EPIC or similar charting systems.
