Job Description

Provide skill building supports to individuals in the performance of daily and independent living skills within their home and community as outlined in the home and community based services policies and procedures;

· Plan, perform, or assist with cares or activities with individuals according to their preference or needs, to include, but not limited to, household chores (cooking, cleaning, personal hygiene, laundry, dishes, vacuuming, fixing bed, household maintenance, etc.), community involvement, social, recreation and leisure activities, communication, personal safety, and behavior.

· Provide opportunities for new experiences in the community or home.

· Focus on the individual rather than the disability.

· Follow, implement, and provide feedback with person-centered and behavioral outcomes.

· Maintain professional boundaries between team members, families and individuals served.

· Provide guidance and direction while being a positive role model to the individual.

· Assist in problem-solving issues.

· Be creative by trying new ideas that create a more positive impact for the individuals receiving services.

· Provide transportation to recreational activities, doctor's appointments, and employment; etc. and/or support individuals with acquiring transportation as needed.

· Look for ways to promote and improve the quality of Second Chances Comprehensive Services.

· Complete progress notes/flow charts, service outcomes and support plans, according to outcomes and supports of individuals

· Support individual's receiving services in planning and structuring activities.

· Bring your own skills, strengths and interests to supporting individuals' activities.

· Support individuals receiving services to exercise their rights and responsibilities in all areas of their lives.

Knowledge, Skills and Abilities:

• High school diploma with one-year experience in the Human Services field preferred..

• Must have a valid Virginia driver's license and current DMV driving record is required and must submit (good driving history).

• Must be able to pass a Criminal Background Investigation and fingerprint clearance

• Must pass Registry Check

• CPR, First Aid, Medication Management and Direct Support Professional training required will be offered as well.

• Compassionate, initiative, task oriented
