Job summary
We are looking to hire a dynamic Licensed Practical Nurse (LPN) to join our phenomenal team of Clinicians. The LPN will supplement nursing care needs of the home health care and/or hospice patient according to the developed plan of care and under the direction of an RN Field Case Manager.- Carries out treatments, teaching, and observation and assessment activities as described in the Plan of Care and delegated by the case manager
- Maintains records and reports according to regulatory requirements and company policies and procedures
- Provides daily report to Clinical Manager and Case Manager to review status of cases; participates in monthly clinical record review
