- Provide In-Home services to clients by partnering with the referral agency, Children, Youth, and Families (CYF) to identify and achieve both client and agency goals.

- Participate in the In-Home Services On-Call Cycle by taking part in the assigned rotation, opening cases, and assisting clients in an emergency or crisis situation.
- Providing the agency with outcomes by conducting North Carolina Family Assessments with all assigned cases that are applicable.

- Act 33/34, Criminal Background, Physical, and FBI Clearances.
- Familiarity with the human service field, the juvenile court system, child development, conflict resolution, and experience with individual and family counseling.
- The ability to maintain client, program, and agency information confidentially.
- Effective verbal and written communication skills.

Job Summary: Case Managers are responsible for providing In-Home crisis and non-crisis case management services to Agency clients.

We use people-centered planning to explore each client's desires, values, hopes, and dreams. For many of our clients, this may entail a stable job within the community or a healthy home where their family can learn and grow. Each client has individual desires and gifts, and we work alongside them to find the best fit for their unique situation.
