    Shift Details: 8 hour shifts, one on-call shift per week, one weekend call per month

    Department: Spiritual Care | Reports To: Spiritual Care Manager

    Summary:

    Performs spiritual care functions associated with the beliefs and practices of one's religious affiliation, as well as being able to provide spiritual and moral guidance and assistance to patients, patient's families and staff members of all cultures, beliefs, values and religious affiliations. Provides spiritual care to acute, palliative, and hospice patients as well as their support persons. Visits patients as soon as possible after admission to the hospital and/or hospice program. The Chaplain must be clinically trained to notice and to address himself/herself to the various spiritual dynamics, feelings and emotions that a patient makes known in verbal and non-verbal communication.

    Responsibilities:

    Essential Job Duties and Responsibilities:

    * Articulates a theological perspective and conveys religious knowledge in particular patient situations and educates hospital staff in meeting the spiritual needs of patients and families.
    * Functions pastorally in a manner that respects the physical, emotional and spiritual boundaries of others.
    * Uses authority appropriately in the provision of Spiritual Care to patients, family members and staff members. Able to identify personal professional strengths and limitations in the provision of spiritual care.
    * Functions within a professional code of ethics (including, but not limited to the Faith Regional Health Services Code of Ethical Behavior and the Code of Ethics of the professional association in which the chaplain holds current membership (or a modified version of such code if the chaplain is not a member of a professional association).
    * Establishes, deepens and ends relationships with sensitivity, openness and respect.
    * Provides effective spiritual and emotional support that contributes to well-being of patients, their families and staff. (Including the triage and management of crises and the provision of spiritual care to persons experiencing trauma, loss and/or grief).
    * Provides spiritual care that respects diversity and differences including, but not limited to age, culture, gender, sexual orientation and spiritual/religious beliefs and practices.
    * Establishes and maintains professional and interdisciplinary relationships.
    * Documents contribution of care effectively in the appropriate records.
    * Facilitates group processes such as family meetings and support groups.
    * Formulates and utilizes spiritual assessments, interventions, outcomes and care plans in order to contribute effectively to the well-being of the person receiving care.
    * Contributes to the overall effectiveness of the Spiritual Care Department, including, but not limited to:

  • Writing articles for publication in FRHS newsletters and bulletins.
    * Leading worship and prayer opportunities.
    * Distributing Bibles and appropriate religious materials.
    * Assisting in maintaining effective information and referral system for patients, families, staff and parish clergy.
    * Keeping accurate records of spiritual care activities.
    * Being available for rotating on-call duties for weekdays, weekends and holidays.
    * Providing sacramental ministry consistent with the practice of employee's religious affiliation.

    * Provides spiritual care and comfort to patients, patient families, FRHS staff and medical staff, following department policies and procedures.
    * Knowledge of general home health care and hospice policies, including, but not limited to:

  • General eligibility requirements for Hospice and Home Health Care services.
    * Infection control practices in the home setting
    * Patient safety concerns in the home setting.

    * Provides spiritual care services to appropriate candidates per protocol (including but not limited to: assessing and addressing of spiritual needs and resources, participating in care conferences, and contributing to bereavement care).
    * Performs written and verbal communications in an accurate and timely fashion with the Health Care Team regarding a patient's plan of care.

    Hours will be dependent on patient census and workload. Ability and willingness to work a flexible schedule, to include after-hours and weekends as necessary.

    Job Requirements:

    The requirements listed below must be representative of the knowledge, skills, minimum education, training, licensure, experience, and/or ability required.

    EDUCATION

    Bachelor's Degree required.

    EDUCATION FIELD OF STUDY

    Successful completion of at least one unit of Clinical Pastoral Education (CPE) preferred.

    Board Certification with the National Association of Catholic Chaplains or the Association of Professional Chaplains preferred.

    EXPERIENCE

    Previous healthcare and/or clinical experience preferred.

    1-3 years of previous experience in Clinical Spiritual Care preferred.

    SKILLS

    Language Skills - Ability to read, write, speak, and understand fluently the English language required.

    Ability to prioritize and manage multiple tasks with efficiency required.

    Excellent customer service skills required.

    Computer/Technology skills for patient charting required.

    Other Certifications/Requirements:

    Good standing with and/or endorsed by the employee's religious affiliation required.

    Current, valid driver's license issued in the state of legal residence required.

    Use of employee's personal vehicle to drive within a range of 50 miles from the facility to job assignments required.
