Job Summary
We are seeking an occupational therapist to join our inpatient flexible workforce team and support adult acute care needs at M Health Fairview University of Minnesota Medical Center East Bank and West Bank Hospitals in Minneapolis and support post-acute care needs at M Health Fairview University of Minnesota Acute Rehab and Transitional Care Unit in Minneapolis.As an occupational therapist partners with the patient and family while performing the Evaluation/Re-evaluation gathers pertinent data performs examination determines treatment diagnosis develops an individualized plan of care through coordination with the patient and family analyzes data to identify needs and potential risks determines prognosis for recovery establishes individualized goals with patient and/or caregiver implements modifies plan of care based on reassessment and patient response coordinates care communicates effectively with interdisciplinary team for all aspects of patient care establishes appropriate discharge plan partners patients families provide skilled intervention alters treatment/plan of care reflect change in status response treatment assesses caregiver learning need provides education documents/bills using acceptable medical terminology follows professional practice standards completes documentation timely manner per department standards compiles billing departmental standards prioritizes tasks/care effective time management understands follows licensure/certification requirements directs supervises as designated pursues personal development clinical best practices meets exceeds productivity standard identifies participates supports quality improvement initiatives
